Michigan Licensed Cardiology Nurse Navigator

Job Summary

To support a growing cardiology practice, the full-time Michigan Licensed Cardiology Nurse Navigator will engage with members virtually, conduct clinical assessments, and facilitate care discussions while managing escalations in a remote setting.

Key responsibilities

• Actively engage with members to address clinical issues and manage escalations, ensuring empathetic communication and care coordination
• Conduct comprehensive clinical assessments and oversee member health under the guidance of the Nurse Team Lead or NP
• Familiarize with Karoo Health systems and conduct a minimum of 20 member calls per day while adhering to documentation standards

Required qualifications

• BS in Nursing and a compact unrestricted Michigan Registered Nurse license
• Five years of nursing experience, including at least one year in cardiology nursing
• Care management experience
• Virtual or telehealth experience preferred
