Michigan Licensed Care Coordinator

Job Summary

Working remotely on a PRN basis, the Michigan Licensed Care Coordinator will assist care managers in ensuring patients receive necessary care, understand their medical conditions, and facilitate communication between patients and the healthcare system.

Key responsibilities

• Collaborate with care managers to ensure patients comprehend their medical conditions and treatment instructions
• Perform transitional care calls, medication reconciliation, and accurate documentation of medical records
• Schedule appointments and enter provider orders in a timely manner

Required qualifications

• Certification as a medical assistant, with preference for certification from the American Association of Medical Assistants
• Current Practical Nurse license in the State of Michigan for LPN candidates
• BCLS/CPR certification preferred
• Graduate of an accredited LPN program for LPN candidates
