Michigan Licensed Care Coordinator RN

Job Summary

A company is looking for a Care Coordinator Registered Nurse - Remote in Michigan.

Key Responsibilities

• Provide telephonic and face-to-face comprehensive assessment and care management services to patients
• Utilize multi-dimensional assessment skills and screening tools to identify high-risk and vulnerable populations
• Coordinate care transitions, including medication reconciliation and follow-up appointments

Required Qualifications

• Valid unrestricted RN license in Michigan
• Three years of clinical nursing experience with chronically ill patients
• Extensive knowledge of chronic care and geriatrics

Preferred Qualifications

• RN, BSN degree
• Experience in a health plan or Physician Organization environment focused on care coordination
• Telephonic care management experience
• Home care and/or hospice experience
• Completion of a Complex Care Management course or CCM certification
