Michigan Licensed Care Manager

Job Summary

A company is looking for a Care Manager II.

Key Responsibilities

• Assess members' care coordination and management needs through comprehensive evaluations
• Develop and implement individualized care plans, coordinating with various health services
• Document progress and facilitate communication among members, caregivers, and healthcare providers

Required Qualifications, Training, and Education

• Bachelor's Degree in Nursing is preferred; active RN license in Michigan is required
• 2 to 3 years of Labor and Delivery clinical experience required
• 3 to 5 years of Case Management experience preferred
• Certified Case Manager (CCM) is preferred but not required
• Current and unrestricted Registered Nurse (RN) licensure in Michigan is required
