Michigan Licensed Care Manager
Job Summary
To support high-quality healthcare outcomes, the full-time Michigan Licensed Care Manager will develop and facilitate complex care management activities for members with physical needs, working remotely Monday through Friday from 8 am to 5 pm Eastern.
Key responsibilities
- Evaluate member needs and barriers to care, recommending personalized care plans and interventions
- Develop ongoing care plans in collaboration with providers and community resources to address unmet needs
- Monitor member status and progress towards care plan goals, providing education and support to members and their families
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Unrestricted Michigan RN license required
- Experience in developing care plans and collaborating with healthcare providers
- Knowledge of social determinants of health and community resources
