Job Summary
To support patient care coordination, the part-time Michigan Licensed Care Team Coordinator will work remotely as needed, assisting with patient interactions, medication reconciliation, and appointment scheduling while ensuring clear communication between patients and the healthcare system.
Key responsibilities
- Collaborate with care managers to ensure patients understand their medical conditions and instructions
- Provide friendly and professional interactions with patients and families, maintaining composure and tact
- Document medical records, enter provider orders, and assist with scheduling appointments
Required qualifications
- Certification as a medical assistant or current Practical Nurse license in Michigan
- Preferred certification with the American Association of Medical Assistants
- BCLS/CPR certification preferred
- Graduate of an accredited LPN program (if applicable)
- Ability to work PRN, Monday through Friday during daytime hours
