Michigan Licensed Case Manager RN

Job Summary

A company is looking for a Case Manager (RN) to coordinate care management for a diverse health plan population.

Key Responsibilities

• Lead a multidisciplinary team to provide holistic care through telephonic and digital communication
• Use the case management process to create, implement, and evaluate individualized care plans
• Coordinate resources and advocate for members to improve health outcomes and ensure continuity of care

Required Qualifications

• Nursing Diploma or Associate's degree in nursing required; Bachelor's degree preferred
• 3 years of clinical nursing experience in various care settings required
• 1 year of case management experience in a managed care setting preferred
• Current, active Michigan Registered Nurse license required
• Certification in Case Management (CCM) required or to be obtained within 18 months of hire
