Job Summary

Seeking a Michigan Licensed Family Nurse Practitioner, the full-time remote position will conduct virtual wellness visits, health assessments, and provide comprehensive patient care while leveraging modern technology to enhance healthcare delivery.

Key responsibilities

• Conduct virtual adult and pediatric wellness visits, including health risk assessments and medication reviews
• Collaborate with in-home clinical team members to evaluate vital signs and coordinate care
• Provide comprehensive care management, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Michigan
• Proven experience performing wellness visits
• Technologically savvy with the ability to use laptops and mobile devices for patient care
• Ability to pass a national background check and valid clinical license search
