Job Summary
A company is looking for a Full-Time Home Health Clinical Intake RN Coordinator (Remote).
Key Responsibilities
- Gather necessary information from referral sources and promote agency services
- Evaluate referrals for client appropriateness for home health services
- Receive and document medical orders, processing referrals accurately and timely
Required Qualifications
- Current Registered Nurse licensure in the state of practice
- Graduate of an approved Nursing education program
- 2+ years of Home Health RN experience
- Proficiency with Epic and/or Home Care EMR Software
- One year of professional nursing experience in California by regulation
