Job Summary

A company is looking for a MSW Inpatient Case Manager - Contingent.

Key Responsibilities

• Provide therapeutic intervention and social work services to patients and families
• Conduct patient assessments and develop care plans while coordinating with providers
• Serve as a patient advocate to facilitate continuity of care in the community

Required Qualifications

• Master's degree in Social Work
• Unrestricted LMSW licensure in the state of Michigan
• Minimum of three (3) years clinical experience
• Certification in Case Management (CCM) or Accredited Case Manager (ACM) preferred
• Self-motivated and able to work in a fast-paced environment
