Michigan Licensed Nurse Care Manager

Job Summary

A company is looking for a Nurse Care Manager (RN/LPN) to work remotely in Michigan.

Key Responsibilities

• Perform telephonic encounters with patients and develop detailed care plans
• Manage a patient panel of at least 100 patients per month, completing encounters for 90% of assigned patients
• Attend mandatory meetings to define the current scope of work and improve chronic care management

Required Qualifications

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
• Current active nursing license in Michigan with no disciplinary actions
• A minimum of two years of clinical experience in Med/Surg, Case Management, or home health care
• Hands-on experience with Electronic Medical Records and familiarity with Microsoft Office applications
• High-speed internet and a compatible desktop or laptop computer (no Chromebooks or iPads)
