Michigan Licensed Nurse Care Manager
Job Summary
Providing ongoing remote care management, the full-time Michigan Licensed Nurse Care Manager will monitor patient vitals, develop individualized care plans, and deliver education and coaching to improve patient outcomes while collaborating with physicians and cross-functional teams.
Key Responsibilities
- Monitor and interpret patient vitals, assessments, and alerts within the care management platform
- Develop, implement, and adjust individualized care plans addressing clinical and social needs
- Deliver patient education and coaching via telephone and secure messaging to empower chronic condition management
Required Qualifications
- Bachelor's degree in nursing or a closely related healthcare field
- Active and valid registered nurse (RN) license in good standing in the state of Michigan
- Verbal and written proficiency in Spanish
- Approximately two (2) to five (5) years of direct patient care experience in medical-surgical or home care
- Ability to work independently in a remote, technology-enabled environment
