Job Summary

A company is looking for a Physical Therapy Assistant (Care Navigator) to support patient success through remote monitoring and intervention.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise and functional training through a technology platform
• Maintain records and feedback on patient progress while assisting in process improvement

Required Qualifications

• 2+ years of experience in musculoskeletal health is a plus
• Graduated from an accredited physical therapy program
• Active Michigan PTA license
• Tech-savvy with access to a newer model laptop and smartphone
• Experience in behavioral modification and Health Coach Certification is a plus
