Job Summary

A company is looking for a Care Navigator (Physical Therapy Assistant) to support patient success through remote monitoring and intervention.

Key Responsibilities

• Provide remote monitoring services to assist patients with their home-exercise therapy program
• Direct users in therapeutic exercise, neuromuscular re-education, and functional training via the technology platform
• Maintain records and feedback on patient progress, while managing a caseload efficiently

Required Qualifications

• Graduated from an accredited physical therapy program
• Active Michigan PTA license
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with access to a newer model laptop and smartphone
• Experience in behavioral modification or Health Coach Certification is a plus
