Job Summary
To support integrated care initiatives, the full-time Michigan Licensed Population Health Specialist will coordinate outreach to patients, assist with care management calls, and collaborate with healthcare teams while working remotely from Lansing, MI.
Key responsibilities:
- Provide proactive outreach to assigned patients and document interactions for team updates
- Coordinate care with Registered Nurse Care Facilitators for patients with complex needs
- Advocate for patients by connecting them with community resources and negotiating with insurance companies
Required qualifications:
- High school diploma or GED; completion of a Medical Assistant program
- Minimum of 1 year of medical or case management experience
- Completion of a nationally accredited program in self-management support concepts within 12 months of hire
- Certified Medical Assistant and/or national case management certification preferred
- Proficiency in Microsoft Excel and experience with electronic medical records preferred
