Michigan Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with Michigan and Compact Licensure.

Key Responsibilities

• Manage patient census with a focus on closing gaps in care
• Identify and coordinate community resources for patient care
• Provide education on chronic condition management and perform medication management

Required Qualifications

• Active Michigan and Compact/Multistate License (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote environment with high-speed Wi-Fi
• Experience with Epic and additional state licensures are a plus
