Michigan Licensed Practical Nurse

Job Summary

A company is looking for a Remote Licensed Practical Nurse (LPN) with Michigan and Compact Licensure required.

Key Responsibilities:

• Manage patient census to close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources beneficial for patient care
• Provide patient education on managing chronic conditions and perform medication management

Required Qualifications:

• Active Michigan and Multistate/Compact License (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Experience in chronic care management
• Ability to work in a remote, HIPAA-compliant environment
