Job Summary

As a Michigan Licensed RN, the full-time Health and Social Services Coordinator will telecommute to analyze and implement individualized care strategies, advocate for member needs, and facilitate access to social service programs.

Key responsibilities:

• Analyze, assess, plan, and implement care strategies tailored to patient needs
• Identify and initiate referrals for various social service programs to support members
• Act as a single point of contact to manage care plans and advocate for patients throughout the continuum of care

Required qualifications:

• Registered Nurse license in the state of Michigan
• 4+ years of clinical experience in a healthcare setting
• 2+ years of Case Management experience
• 2+ years of Medicaid experience
• 1+ years of experience with MS Word, Excel, and Outlook
