Michigan Licensed RN Case Manager

Job Summary

A company is looking for a RN Client Focused Case Manager (Remote) to support patients through case management.

Key Responsibilities

• Assess clinical and social determinants of health to identify healthcare service needs
• Conduct comprehensive assessments and develop patient-centered care plans in collaboration with healthcare teams
• Monitor and evaluate the effectiveness of interventions, adjusting care plans as necessary

Required Qualifications

• A degree in nursing is required; a Bachelor's degree in nursing is preferred
• Minimum of 2 years of experience in the healthcare industry, preferably in a health plan setting
• Experience in Case Management is required
• General understanding of Medicare and Medicaid regulations is required
• Registered Nurse (RN) with a valid, unrestricted State of Michigan license and Multi-State Licensure is required
