Job Summary

A company is looking for a Remote RN Case Manager with Michigan Licensure.

Key Responsibilities

• Manage a caseload of members, facilitating high-quality, cost-effective patient outcomes
• Educate members on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program
• Coordinate care among patients, healthcare providers, and support networks, ensuring compliance with regulations and policies

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
