Michigan Licensed RN/Social Worker Case Manager

Job Summary

A company is looking for an RN/Social Worker Case Manager-Ambulatory.

Key Responsibilities

• Conduct comprehensive assessments of patients and their families to identify health care needs
• Develop personalized care plans and engage patients in their care through advocacy and education
• Coordinate transitions between care settings and collaborate with treatment teams to optimize patient outcomes

Required Qualifications

• Bachelor's degree in nursing or a related field, or a Master's degree in Social Work
• Minimum of three years of clinical experience
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid Michigan license
• Certification in Case Management (CCM) or Accredited Case Manager (ACM) required within three years of hire
• Experience in discharge planning, home health care, or community health preferred
