Michigan RN Care Navigator

Job Summary

Managing a caseload of patients, the full-time Michigan RN Care Navigator will provide case management focused on kidney health, facilitating care coordination and promoting quality outcomes in a 100% remote environment.

Key responsibilities

• Handle inbound and outbound calls to deliver exceptional service to members
• Educate members on kidney health and related conditions, including dialysis and transplant options
• Collaborate with healthcare providers to coordinate care and address patient needs

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
