Job Summary

Key responsibilities

• Accesses, analyzes, plans, and provides follow-up care while coordinating transitions across the healthcare continuum
• Facilitates timely referrals and fosters positive relationships with community providers
• Evaluates patient progress and documents care status through various communication methods

Required qualifications

• 3 years of specialty experience in care coordination or 5 years of clinical experience as a licensed RN
• Current Minnesota Registered Nurse (RN) License
• Basic Life Support certification
