Minnesota Licensed Care Coordinator RN

Job Summary

Supporting the Endocrine Department at M Health Fairview Clinics and Surgery Center in Maple Grove, the full-time Minnesota Licensed Care Coordinator RN will manage patient care coordination, facilitate referrals, and provide education while working primarily onsite with potential for hybrid scheduling after training.

Key responsibilities

• Accesses, analyzes, plans, and provides follow-up care for patients within a specific medical specialty
• Facilitates timely referrals and collaborates with community providers to ensure coordinated transitions of care
• Evaluates patient progress and documents care while serving as a resource and educator for patients and medical staff

Required qualifications

• 3 years of specialty experience in care coordination or 5 years of clinical RN experience with the ability to learn a new specialty area
• Current Minnesota Registered Nurse (RN) License
• Basic Life Support certification from the American Heart Association or Red Cross
