Job Summary

Key Responsibilities

Receives referrals and conducts comprehensive clinical assessments to determine member needs
Collaborates with members and families to develop and monitor effective care plans
Identifies and facilitates access to relevant resources and ensures proper documentation of member data

Required Qualifications, Training, and Education

Registered Nurse with psychiatric experience or a licensed behavioral health professional
Minimum of 3 years of relevant direct clinical care experience
CCM Certification or ability to obtain within 3 years of employment
Flexibility to work varied hours
Proficiency in computer applications
