Job Summary
A company is looking for a Case Manager - RN.
Key Responsibilities
- Receives referrals and conducts comprehensive clinical assessments to determine member needs
- Collaborates with members and families to develop and monitor effective care plans
- Identifies and facilitates access to relevant resources and ensures proper documentation of member data
Required Qualifications, Training, and Education
- Registered Nurse with psychiatric experience or a licensed behavioral health professional
- Minimum of 3 years of relevant direct clinical care experience
- CCM Certification or ability to obtain within 3 years of employment
- Flexibility to work varied hours
- Proficiency in computer applications
