Job Summary
To support a growing telehealth initiative, the part-time Minnesota Licensed Clinical Pharmacist will deliver Comprehensive Medication Management (CMM) services remotely, helping clients with complex medication regimens and ensuring actionable next steps for their health journey.
Key responsibilities
- Provide telephonic CMM consultations while balancing clinical quality and productivity goals
- Document interactions accurately and maintain accountability for consultation volumes and follow-ups
- Collaborate with team members to integrate medication management into participants' broader care journeys
Required qualifications
- Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited pharmacy school
- Licensed Pharmacist in any state in good standing, with the ability to obtain a Minnesota license within 3-6 months of hire
- 2+ years of experience providing Comprehensive Medication Management (CMM) and/or MTM
- 5+ years as a Clinical Pharmacist in inpatient and outpatient settings
- 2+ years of supervisory or lead experience in remote pharmacy advisory services
