Job Summary
To enhance patient outcomes in a fully remote capacity, the Minnesota Licensed Clinical Pharmacist will evaluate pharmacotherapy, monitor drug therapy, and deliver patient education while collaborating with interdisciplinary teams and participating in quality improvement initiatives.
Key responsibilities
- Oversee therapy management, addressing medication errors and adverse reactions in accordance with organizational policies
- Support medication use processes to ensure safety, efficacy, and compliance with guidelines, while maintaining accurate records of clinical interventions
- Participate in initiatives aimed at enhancing medication safety, operational efficiency, and cost-effectiveness
Required qualifications
- Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy
- Current Pharmacist license as granted by the appropriate state licensing authority obtained within 2 months of hire date
- American Heart Association BLS certification obtained within 6 months of hire date
- Preferred completion of an ASHP accredited PGY1 or specialty residency
- Preferred 3 years of retail or specialty pharmacy experience
