Job Summary

Dedicated to enhancing patient outcomes, the full-time Minnesota Licensed Heart Failure RN will provide bedside care in a hospital setting, manage outpatient clinic visits, and conduct occasional remote work, focusing on education and ongoing disease management for heart failure patients.

Key responsibilities

• Provide bedside care and support for heart failure patients in the hospital
• Manage new and follow-up outpatient clinic visits for heart failure management
• Educate patients on disease management and provide ongoing support across the continuum of care

Required qualifications

• Graduate from an accredited nursing program required
• Current Minnesota RN License required
• BSN or MSN preferred; BSN or higher required within 36 months of hire
• Basic Life Support (BLS) certification from the American Heart Association required
• 1 year of recent cardiology work experience preferred
