Job Summary

A company is looking for a Medication Therapy Management (MTM) Pharmacist.

Key Responsibilities

• Provide comprehensive medication management services to members primarily telephonically and face to face
• Document medication management services for care coordination with members and their care teams
• Participate in cross-functional care team meetings to improve clinical programs and enhance processes

Required Qualifications, Training, and Education

• Doctorate of Pharmacy (PharmD) or equivalent degree
• Current unrestricted Minnesota licensure to practice pharmacy
• Graduated from a college of pharmacy after May 1996 or completed an approved ACPE program
• 3+ years of related professional experience
• Strong clinical pharmacy knowledge
