Job Summary

Offering a flexible per diem opportunity, the 1099 Contracted Nurse Assessor will visit clients at their residences to complete assessments of their medical, functional, and cognitive status, with the ability to schedule visits based on personal availability and client needs.

Key responsibilities

• Obtain thorough cognitive and health assessment information as required
• Accurately complete assessments and comply with all assessment instructions
• Communicate promptly with the organization regarding file status and appointment times

Required qualifications

• Current Minnesota Registered Nurse license
• Ability to perform assessments within a 75-mile radius
• Experience in health assessments or related fields
• Access to a mobile device or computer for electronic assessments
• Flexible schedule availability, including mornings, afternoons, and evenings
