Job Summary
To support oncology patients, the casual remote Minnesota Licensed Oncology RN Navigator will manage care navigation, communication, and education for patients from referral to initial consultation with the cancer care team.
Key responsibilities
- Navigates patient care from initial referral to oncology until consultation with the care team
- Coordinates care for patients new to cancer care and collaborates with the multidisciplinary team to develop initial plans of care
- Assesses patient clinical history and facilitates scheduling of initial appointments while tracking referrals and support services
Required qualifications
- Associates or Bachelor's Degree in Nursing
- 3 years of adult clinical oncology nursing experience
- MN Registered Nurse (RN) License
- Oncology Certified Nurse within 1-1/2 years
