Job Summary

To support a growing practice, the fully remote Minnesota Licensed Outpatient Therapist will collaborate with a multidisciplinary team to provide therapeutic services to the senior population, utilizing evidence-based approaches and ensuring compliant documentation.

Key responsibilities

• Collaborate with a psychiatric physician, RN Director, and medical office coordinator to deliver services to seniors
• Develop, personalize, and update treatment plans using evidence-based therapeutic approaches
• Provide compliant documentation based on Medicare guidelines within designated time-frames

Required qualifications

• Valid Minnesota licensure in Social Work or Counseling (e.g., LGSW, LISW, LCSW, LPC, LPCC, LMFT)
