Job Summary

A company is looking for a Pharmacist, MTM and Population Health.

Key Responsibilities

• Improve population-based and patient-specific medication outcomes through collaboration with a multi-disciplinary medical team
• Support population health initiatives and improve Medicare Star measures related to medication use
• Provide direct patient care through comprehensive medication management once credentialed

Required Qualifications

• Graduated from an accredited college of pharmacy or completed an approved education program
• Two years of recent clinical pharmacy experience in ambulatory care or completion of a specialized post-doctoral residency
• Current Minnesota Pharmacist license in good standing
• Deep knowledge of evidence-based clinical guidelines, pharmacology, and therapeutic disease states
• Board Certified Ambulatory Care Pharmacist (BCACP) or Board-Certified Pharmacotherapy Specialist (BCPS) preferred
