Job Summary

As a Care Navigator, the full-time Minnesota Licensed Physical Therapy Assistant will provide remote monitoring services, assist patients with their home-exercise therapy programs, and maintain communication with evaluating clinicians to support patient recovery and engagement.

Key responsibilities

• Provide remote monitoring and support for patients during their home-exercise therapy programs
• Direct and assist users in therapeutic exercises and functional training through a technology platform
• Maintain necessary records and provide ongoing feedback on patient progress to clinicians

Required qualifications

• Active PTA license in Minnesota
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Proficiency in using multiple technology platforms (EHR, messaging, scheduling)
• Experience in behavioral modification or Health Coach Certification is a plus
