Job Summary
As a Care Navigator, the full-time Minnesota Licensed Physical Therapy Assistant will provide remote monitoring services, assist patients with their home-exercise therapy programs, and maintain communication with evaluating clinicians to support patient recovery and engagement.
Key responsibilities
- Provide remote monitoring and support for patients during their home-exercise therapy programs
- Direct and assist users in therapeutic exercises and functional training through a technology platform
- Maintain necessary records and provide ongoing feedback on patient progress to clinicians
Required qualifications
- Active PTA license in Minnesota
- Graduated from an accredited physical therapy program
- 2+ years of experience in musculoskeletal health is a plus
- Proficiency in using multiple technology platforms (EHR, messaging, scheduling)
- Experience in behavioral modification or Health Coach Certification is a plus
