Job Summary

A company is looking for a Value-based Care Registered Nurse (Per Diem).

Key Responsibilities

• Provide Transitional Care Management and Triage for women ages 65 and up
• Educate patients on health conditions and facilitate continuity of care among team members
• Respond to patient inquiries and manage medication reconciliation and adherence

Required Qualifications

• Active, unrestricted Minnesota RN license or ability to obtain by start date
• Minimum of 2 years' experience as an RN in a clinical setting
• Prior experience as a triage RN or RN case manager
• Ability to collaborate effectively with care team members
• Experience using electronic health records
