Key Responsibilities

Engage members, families, and providers in complex case management through telephonic outreach
Conduct clinical assessments and collaborate with the interdisciplinary care team to develop comprehensive care plans
Monitor and update care plans while ensuring compliance with documentation standards and promoting cost-effective outcomes

Required Qualifications and Education

Registered nurse with a current Minnesota license and no restrictions
5 years of relevant clinical care experience
CCM Certificate or ability to obtain within 3 years of starting the position
Experience in health plan operations, including networks and benefits
Proficiency in computer applications
