Job Summary

A company is looking for an RN Case Manager - Care Advocacy.

Key Responsibilities

• Serve as a customer advocate by identifying needs and guiding members to appropriate resources
• Conduct comprehensive clinical assessments and develop plans of care to address gaps in care
• Collaborate with members, families, and healthcare providers to ensure effective care coordination and positive clinical outcomes

Required Qualifications

• Registered nurse with a current MN license and no restrictions
• 5+ years of related professional experience
• CCM Certification or ability to obtain within 3 years
• High school diploma (or equivalency) and legal authorization to work in the U.S
• Computer application proficiency
