Key Responsibilities

Conducts clinical assessments and develops comprehensive care plans for members with complex medical conditions
Collaborates with healthcare teams to ensure effective communication and positive clinical outcomes
Monitors and updates care plans while identifying cost-saving opportunities and community resources

Required Qualifications, Training, and Education

Registered nurse with a current MN license and no restrictions
5 years of relevant clinical care experience
CCM Certificate or ability to obtain within 3 years of starting the position
Experience in managed care is preferred
Proficiency in computer applications and telephonic communication
