Job Summary

To support the Gastroenterology team, the part-time Minnesota Licensed RN Navigator will provide care navigation, communication, and education to patients and families in a fully remote role, working Monday through Friday with no weekend or holiday requirements.

Key responsibilities

Navigates patient care from initial referral until consultation with the care team
Coordinates care for patients new to the specialty and assists with scheduling initial appointments
Educates patients and families on additional testing and develops a plan of care based on clinical history

Required qualifications

Associates Degree in Nursing or Bachelor's Degree in Nursing
3 years nursing experience within a specific patient population group
MN Registered Nurse (RN) License
