Job Summary

A company is looking for an RN Specialist for Complex Case Management.

Key Responsibilities

• Engage members with high-cost, complex medical conditions through telephonic outreach and clinical assessments
• Collaborate with the interdisciplinary care team to develop and monitor comprehensive care plans
• Identify and facilitate referrals to relevant resources and ensure documentation meets regulatory standards

Required Qualifications, Training, and Education

• Registered nurse with a current MN license and no restrictions
• Five years of relevant clinical care experience
• CCM Certificate or ability to obtain within three years of starting in the position
• Experience in health plan operations and case management
• Proficiency in computer applications
