Job Summary

Key Responsibilities

Engages members with high-cost, complex medical conditions through telephonic outreach and clinical assessments
Collaborates with the interdisciplinary care team to develop and monitor comprehensive plans of care
Identifies cost-saving opportunities and facilitates referrals to relevant resources

Required Qualifications, Training, and Education

Current MN RN license with no restrictions
5 years of relevant clinical care experience
CCM Certificate or ability to obtain within 3 years of starting
Experience in managed care preferred (2+ years)
Proficiency in computer applications
