Minnesota Licensed Telephonic Case Manager

Job Summary

To support individuals recovering from workplace injuries, the full-time Minnesota Licensed Telephonic Case Manager will collaborate with healthcare providers and employers to create and implement effective treatment plans while working remotely.

Key responsibilities

• Manage and assess treatment plans for injured or disabled individuals, ensuring appropriate healthcare services are provided
• Collaborate with treating physicians and employers to facilitate early return-to-work modifications
• Independently prioritize and manage a caseload, preparing status updates and assisting in training new staff as needed

Required qualifications

• Active Minnesota Registered Nurse (RN) license in good standing
• URAC-recognized certification in case management (e.g., CCM, CDMS, CRC)
• Associate's or Bachelor's Degree in Nursing or related field
• 2+ years of clinical practice experience, preferably in workers' compensation
• Reliable internet access for remote work
