Minnesota Licensed Triage Nurse

Job Summary

Providing telephone triage support for Assisted Living and Hospice care, the part-time Minnesota Licensed Triage Nurse will work remotely every third weekend from 7am to 3pm, ensuring high-quality patient care through assessment, documentation, and follow-up on patient needs.

Key responsibilities

• Independently provide professional nursing services over the telephone, documenting symptoms, assessments, and advice given
• Follow up on high-risk situations and analyze data to develop solutions for improved service outcomes
• Maintain clinical standards and ethics while documenting efficiently in electronic health records

Required qualifications

• Active Registered Nurse license in Minnesota, or ability to obtain within 60 days of hire
• Minimum of 1 year of care management experience in home care or hospice
• Strong clinical and relational skills with the ability to analyze situations and implement solutions
• Capability to manage multiple priorities and high-stress situations effectively
• Proficiency in Microsoft Office Suite
