Job Summary
To support a growing senior health practice, the part-time Minnesota Licensed Triage Nurse will gather clinical information over the phone, direct healthcare services, and provide self-care advice while working remotely from Minnesota.
Key responsibilities
- Gather and assess clinical information from clients, families, or caregivers to guide healthcare decisions
- Provide triage care and health instructions according to established clinical guidelines and state regulations
- Communicate effectively with healthcare providers and community resources to coordinate client care and ensure continuity
Required qualifications
- Graduate of an accredited school of nursing with a current state nursing license
- One to two years of recent acute care, triage, or home health care experience
- Recent triage experience in a clinic, assisted living facility, or skilled nursing facility
- Proficiency with technology and relevant applications, including clinical software and Microsoft Office
- Verification of COVID vaccine status or an approved exemption
