Job Summary
Providing telephone triage support for Assisted Living and Hospice teams, the part-time Minnesota Licensed Triage Nurse will work from 3pm to 11pm every third weekend, ensuring accurate documentation and high-quality patient care.
Key responsibilities
- Independently provide professional nursing services over the telephone, documenting symptoms, assessments, and advice accurately
- Follow up on high-risk situations and analyze data to develop solutions for improved services and outcomes
- Maintain high clinical standards and document efficiently in electronic health records
Required qualifications
- Must be licensed as a Registered Nurse in Minnesota
- At least 1 year of care management experience in home care or hospice
- Strong clinical and relational skills
- Ability to analyze situations and implement effective solutions
- Proficient with Microsoft Office Suite
