Mississippi Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members.

Key Responsibilities

• Evaluate member needs and recommend care plans to achieve optimal health outcomes
• Coordinate care between members, families, and healthcare providers to ensure person-centered services
• Monitor member progress and update care plans as necessary to meet goals and address unmet needs

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Registered Nurse (RN) State Licensure and/or Compact State Licensure required
• Experience in developing personalized care plans and facilitating member education
• Knowledge of social determinants of health and community resources
