Mississippi Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Coordinate care between members, families, and providers to ensure person-centered services
• Monitor member progress and update care plans as necessary to meet goals and needs

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse State Licensure and/or Compact State Licensure required
• Must reside in or around Mississippi for remote position
