Mississippi Licensed Care Manager

Job Summary

A company is looking for a Care Manager to collaborate with interdisciplinary teams and community organizations to enhance member care and outcomes.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Coordinate communication and collaboration among the care team to achieve member goals
• Monitor and evaluate member progress and satisfaction with care plans

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or Bachelor's degree in a healthcare field or equivalent experience
• Preferred advanced degree associated with clinical licensure
• Minimum of three years of experience in nursing, social work, or healthcare professions
• Preferred experience in Medicaid and/or Medicare managed care
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor
