Job Summary
To provide high-quality, cost-effective healthcare outcomes, the full-time Mississippi Licensed Care Manager will develop and facilitate complex care management activities for members with physical needs, working remotely while collaborating with healthcare providers and community resources.
Key responsibilities
- Evaluate member needs and barriers to care, recommending personalized care plans and interventions
- Coordinate and collaborate with providers and caregivers to ensure timely access to services and monitor member progress
- Educate members and families on healthcare options, benefits, and disease processes while maintaining compliance with regulations
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse license in Mississippi or Compact State Licensure
- Experience in developing care plans and collaborating with healthcare providers
- Knowledge of social determinants of health and community resources
