Job Summary
To support a medication optimization program, the full-time remote Mississippi Licensed Clinical Pharmacist will conduct comprehensive medication reviews, engage in telephonic consultations with medical providers, and collaborate with interdisciplinary clinical staff to enhance patient medication adherence and optimize drug therapy.
Key responsibilities
- Perform and document patient-specific comprehensive medication reviews (CMRs) while adhering to productivity and quality standards
- Drive medication interventions and support medical providers in adhering to evidence-based medication guidelines
- Engage directly with patients to improve their understanding and adherence to medication therapy
Required qualifications
- Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD)
- Current, unrestricted pharmacist license in the state of service
- Minimum three years of active pharmacist practice experience, preferably in managed care or clinical patient care
- PGY1 residency in clinical fields is preferred
- Board certification in Medication Management or related clinical field is desired
